Recently, the U.S. government has set forth some rules as
to what health information is private, and when and how it can be disclosed.
Protecting the personal health information (PHI) of our patients is
important to our clinical practice and we endeavor to put forth procedures
to ensure the privacy of your medical record.
First of all, we will use your PHI only as it relates to medical decisions
made by the staff. We will not disclose that information to any other person
or organization without your permission.
ON Semiconductor will receive a list of employees who received a Flu
Shot. The company will not receive any other information
about your health from Physico. We do not bill insurance, therefore we will
not be releasing your PHI to any other third party, except for the
government when required to do so by law. We will keep your PHI in our
secure files for at least seven years. You can access this information by
submitting a signed records release form to the office. Please allow a few
days for this request to be processed.
To ensure your privacy while receiving your immunizations, we will call you
by your first name. We will gather the information needed to administer the
immunization as discretely as possible. Your PHI will either be in the hands
of a Physico staff member, or set on a secure table, facing down. The
immunization will be given behind a private screen if any disrobing is
required. Please wear a shirt with sleeves that can be rolled up so that you
do not have to disrobe.
Physico Health Assessments will abide by all laws, federal and state,
related to the privacy of PHI. We will abide by the terms in this notice
that is posted at the clinic site and available to you, and we will continue
to review our operations to do the very best we can in protecting you or
your health information. If you have any questions or concerns related to
this notice or your privacy rights, contact the office at 480 510-3075. You
may also take your concerns to the Secretary of Health and Human Services if
you believe that your rights have been violated. This office may at times
update this notice, and at that time, you will have access to any changes
that have been made. This notice is current as of September 17, 2007.
