Athletes and Parents
Thank you for allowing Physico Health Assessments to perform your sports physical. We conduct thousands of AIA approved sports physicals statewide and are proud of our excellent community standing.

1.  You may schedule an appointment with our online appointment system if your school has so indicated - see Appointments above.
2.  Some schools are using a paper roster - sign up with the athletic secretary at your school.

You may contact your school if you need clarification.


MAKE SURE YOU:

  • Bring health history form (filled out & signed by parent if < 18 yrs)
  • Bring consent form (must be signed by parent if < 18 yrs)
  • Drink 24 ounces of water before the sports physical
  • Wear glasses/contacts for the physical if you need them
  • Bring $25 - cash only please
